Registration of Interest Form

CDC+ is available only to individuals who receive Medicaid and who are currently receiving services from either the Aged and Disabled Adult Waiver or the Traumatic Brain and Spinal Cord Injury Waiver. These individuals are either elders, physically disabled adults, or individuals with brain/spinal cord injuries and have a current case manager.
